Error when trying to upgrade

This is the error I get when I tried to upgrade:
I put the site into maintenance mode and copied the new files. Then went to the upgrade link and bam! Error city.
Any ideas?

Did you override the concrete/helpers/form.php with a /helpers/form.php in your local install?
Or possibly did you copy the contents of the concrete/* directory into your web root? Typically I upgrade c5 sites by renaming the /concrete directory to /concrete5.3.1 or something, then I copy the new /concrete folder to the site root.
How did you go about upgrading?
