For Server Name put localhost in it
and don't mark your response as best answer when you didn't answer the question.
and what for the rest?, Unable to connect to database.
you have to find that out with your hosting provider, you have to create a database. For you I would try to install through simple scripts or softaculous (or host with c5) because the setup is much easier. You have to create a mysql database with permissions INSERT, SELECT, UPDATE, DELETE, CREATE, DROP, ALTER
Unable to connect to database.

no its not the software. I really would try to install thru simple scripts or softaculous if you can't figure it out.
It sounds as though you can't make the database connection... Who created the database? What hosting provider are you using? Have you setup other content management systems on this server before?
For the benefit of any other readers, I had the same problems when I installed. I gave all of the permissions to my user when I set up the database to start with. I kept getting the failure message after I changed the data in the form on the first page. I solved this by closing the page and re-opening it and entering all of the information again every time I made a change
Entering "localhost" worked perfectly for me. I sure wish that the installer would list such simple and perhaps typically default answers directly from within the installer.
There's an installer... where?
