I setup Concrete in February for the first time. I had some trouble initially (I don't remember what) and when I contacted Bluehost for assistance, they said they don't support manual installations, so I deleted everything and used the SimpleScript and had a successful install of 5.3.3.1.
Now two months later I find I need to upgrade, but I also see there have been issues recently with SimpleScript. Two questions:
1. Is is safe to go directly from 5.3.3.1 to 5.4.0.5
2. Should I use SimpleScript or ignore Bluehost and do it manually?

When you say backup the entire site you mean just the files, correct? There should be no need to backup email and user accounts because the Concrete install doesn't impact those.
Also, I am currently working in a sub-domain, so the installation shouldn't impact anything above the sub-domain root (I am backing up the root anyway as a precaution, but I want to believe I shouldn't need to)

correct - you should need only back up the website files in your subdomain folder. The folder you are going to be replacing is the "concrete" folder that lists right above the "config" folder, with the same folder from the concrete 5.405 ZIP file. You don't need to do anything to the other folders.
FIRST: Bluehost uses CPanel, so first, use cpanel to save a backup your site's database, and then use the file manager to make a ZIP file of your entire site. This way if things go wrong you can have your current site back within minutes.
In terms of versions, you shouldn't have a problem with the upgrade. I would just follow the instructions here and upgrade manually. Make sure after you've replaced the concrete directory you immediately visit the URL listed in the third bullet:
