Transferring site from local machine to server advice

I've successfully installed C5 and installed my own designed site as theme, and got everything working great... locally.

Now I want to upload to my server space. I've looked for guides on the C5 site but can't find walkthroughs to help prepare the site and databases to be run from a server.

Can anyone point me in the right direction?
