Unable to connect to database.

I can't get the database to work, and I thinking the perhaps I'm not getting right in filling the information of the database...

I'm not sure about the server that's the same as host name???

Please provide more information, as to if you are attempting to install concrete5 on your web host or your local host , also would be better if you can mention the details that you are filing in the install form screen.
