Users can no longer register - VERY IMPORTANT

I upgraded to 5.4.1 and am just coming to realize that new users are not able to register for the site (I have 'email must be validated') as the option.
When a new user registers - it shows a blank white page, the username & email shows up in the 'Find Users' area, but no other information is recorded and they do not receive their confirmation email.
This seems to have happened only after upgrading to 5.4.1

I AM sorry for posting before I troubleshoot, but I find it's best to see if there's any help out there while I'm in my turmoil...
There wsa an EXTRA event add-on I was creating a while back to add extra functionality to the registration page. I ended up adding it to an already functioning event thus making it not necessary to be there. When I did the upgrade to 5.4.1 it seemed to have picked up on it being there and throwing an error, while the previous version just ignored it.
Weird, scary, frustrating...but better now.

I changed the setting to registration is enabled (no validation) and when I try to register again it says
"An unexpected error occurred. A database error occurred while processing this request"
