the entire /help section is out dated, which is why it has been hidden

download a new version of concrete5 upload the /concrete directory overriding the old one (make a db backup first) then go to the upgrade url (/index.php/tools/upgrade

Unfortunately that did not work. I downloaded the lastest version over 5.3.1.1. I then went into the admin and still see that I am using 5.3.1.1. I then went to /concrete/tools/required/upgrade. It says: "Your site is already up to date! The current version of Concrete5 is 5.3.1.1. You should remove this file for security."

How does something so mainstream be broken or mis-directed? The document says to either use the updater or go back and update from the next version then download and update each consecutive version. Now I am confused.

5.4.0.5 is the latest not 5.3.1.1

try updating to 5.3.3.1 then to 5.4.0.5

It sounds like you just haven't downloaded the latest to install:

Either that or you're having some errors you're not reporting here.

I've downloaded twice to insure I did not have a incomplete set. Being that the earlier version worked, I am thinking that C5 has imposed more from the PHP config, which is causing it to break. These are brand new installs, not overwriting anything.
