Working fine, then: Fatal error: Uncaught exception 'ADODB_Exception'

It seems I can't add any pages or edit a page.

I get this message:

Is it likely my host changed something and broke my database?

I haven't TOUCHED this site in months. Then, I log in to make an edit and lost everything on that page. If I edit a page, I lose it and that page now becomes the error message. If I create a page, I get that error message. How does a database break?

It seems like it might be related to the weird "approval" process which all of a sudden is popping up. I think this because it says "duplicate key" and when I reload a page, sometimes it says "this page is pending approval"

HOW DO I TURN OFF "PENDING APPROVAL"? I never turned this on to begin with!
