Advanced User Profile Extends & Admin/Client Management Tool

Hi,
I am looking for a good developer to extend the default user profile page and create a dashboard single page that will adjust settings for the client profile.
1) An admin2 (a subadmin) needs the ability to create a new account for a client to use to login.
The information needed for each new client is the same info here:
/index.php/dashboard/users/add/
However, we need to add "Company Name" and "Phone Number" so the fields will be:
Company Name, Create Username, Create Password, Email, Phone Number

2) On this user profile page, the admin2 will be able to pick 2 more checkbox settings "Disability" and "Life Insurance"
Note: All “Highlights and Features” are settings set by the admin/admin2 on a dashboard setting page.
- Disability checkbox when clicked shows a dropdown next to it.
The drop down is going to have choices of Life Insurance companies added by the admin.
Based off the chosen company, another area will appear.
This area will have a list of “Disability Highlights and Features” with their own respective checkboxes.
All checkboxes chosen will appear on the client end page (I have a mockup of this page done).
- Life Insurance checkbox when clicked shows a dropdown next to it:
The drop down is going to have choices of Life Insurance companies added by the admin. Some companies will be different than Disability Life Insurance Companies from above.
Based off the chosen choice, another area will appear.
This area will have a list of "Life Insurance Highlights and Features” with their own respective checkboxes.
All checkboxes chosen will appear on the client end page (same mockup).
Note 2: All checkboxes for "Highlights and Features" can either be a file chosen from the file manager (PDFs, .doc, etc) or content typed via a WYSIWYG.

3) The admin2 will have the ability to upload a logo image on the user-add page.

4) There will be Contact Information that is associated with each Life Insurance Company which will appear on the client end page based on the chosen company.
