Need aspx form turned into php form. Then use form to create custom job application block. I have all current code including mailer

Hi,

I'm redesigning a Job Staffing copy. The company currently has a job application online built in ASP.

Because I am building their new site on C5, I would like the ASPX form and mailer converted into PHP for integration with C5.

Then I would need the form turned into a custom block so the client can add new form field with custom values.

Timeframe is 3-4weeks

Please let me know any questions and what this would cost.
