Need help with my Add Block. It comes out empty every time I try and use it

Hi,
Im needing help to fix my add block on edit mode. When trying to add a block on my scrapbook, or global settings or anywhere throughout the site it does not allow me. Once you click on add block the white window comes up but comes out blank. Not sure what the issue is?

Hi Gio,

I have to check what is the problem therefor i need ftp and admin details.

I will not change anything in admin or in ftp , so don't worry about it.
