Need One-off page or Custom Block

I am needing to build a one-off registration page or a custom block that is a registration form. The block will require no options to be set.
I am looking to build a form that I have a google form representation of. It will be used so people can sign up as a reseller on my site. Upon completing the form, I would like the user to be added to a User Group called "Customers" already created, and then a notification to be sent to the site admin of the user account, so the account can be verified and manually added to the User Group of "Resellers".
This site is set to go live on March 1st so if someone could help me get it completed within 2 weeks would be greatly appreciated.
Please respond with an estimate of the time you'll think you may need to complete this and your compensation rate please.

The login isn't the issue. I need to have two separate registration forms. One is the normal form for general consumers to create an account to make a purchase, and another for resellers to create an account, which will make them both a customer and a reseller.

JB1 has an addon 'register user pro' that may be capable of doing what you want. Worth sending a pm/question to ask.
ProForms has a register user question type that allows you to add them to a group automatically. You can have two forms, each going to it's own group.

There is a login addon that you can put anywhere. Just customize that.
