Who can help me? For a client i am looking for an online checklist for a staging company.

I have a certain flow in mind and i will try to explain it here:

The checklist has to have a front and backend system.

On the front end:

The start of the checklist only includes the following fields:
Clientname:
Date:
homeadress:
email adres:
Telehone number:

After these are filled in, the user can select a room of a house(dropdown), for example: Bathroom, livingroom, kitches etc. If the needed room is not in the list there has to be a possibility for the user to add a custom room to the checklist.

When the room is selected the checklist questions of the specified room wil be shown, user can fill these in and when clicked on a 'end room' button or so, the 'select a room' dropdown appears again and so on.


In the questions per room there has to be some kind of autocomplete, when it consearns an open question. When the answer that the user wants to fill in, is not in the autocomplete list the answar can be filled in anyway and than automaticly has to be added to the autocompletelist.

When the checklist is complete the user hes to press a savebutton wich wil save the form in the backend and will generate a styled PDF showing only the filled in fields of the checklist.

In the back-end there has to be a possibility to add rooms, questions per room (with the answer possibility as dropdown, selectbox and text area), autocomplete list. Oor change one of the above points.

I am aware my english is not so well and that what i am searching for is perhaps a bit unclear but i would really be helpend with an answer if this is possible or not and what it would cost to build it.
