Simple form validations needed. . .

Hello all.
This is my first foray into working with validations in c5's forms, and the documentation isn't enough to get me started, so I figured I would post this one as a job, get it done, and review the code to learn from.
What I need it very simple, and I could do it w/ procedural PHP, but I want to figure out how to utilize c5's form helpers, etc. and get started down that path by seeing how this is done.
Basically, I have a form, created w/ the form block, that has 4 questions, each with a yes/no radio button.
All I need is to validate the buttons. If they're all marked "Yes", then go to another page I already have and include the success message at the top.
If any are marked "No", it fails, and the failure message can be displayed w/ the form like the c5 form block already does.
Simple enough? I need it pretty quickly, and I'm planning on studying how everything is accomplished.
Can't wait to dig into this. Hope someone is up for the ever so challenging task!
