Help to find correct add on

Hello dear,
I would like to open a similar page as below:

I am searching for a add on which you can see on the right side, this add on require from the customer to fill in name and other information and the most important is that they have to choose a date to pick up the product.
And the date function doesn't allow you to put an order if you choose an date which is within around 24 h. In this case you have to put the order a day before to get the product the day after, if you order later in the day, then you will be allowed to pick the product 2 days after and not the day after.
Is it possible for me to get a similar add on?

I have before used prestashop, but never concrete5.
It is my first time, so I am very bad at it.
I don't believe that I can do a similar function on my website without any add on.

The website I have found, they have built the website so you can put an order, but can I do similar but also that my customers can pay for the order directly on my website after they have picked a date for picking up the product?
I don't want the customers to pay in my physical shop.
