Add Block comes out blank and empty PLEASE HELP

Hi, when on my global scrap book when trying to edit or add a new block the page comes out empty.
This is also happening all throughout my site. On editing and want to add a block the page comes out blank and unable to do anything.

PLEASE HELP!!
