Advanced Permissions

This is a strange one. I have enabled advanced permissions in site.php on three sites. All use the same template, all are on the same server, all have the same entry in the site.php file. However on two sites i get the permissions dialogue no problem, but on the third i get what looks like the bottom of the permissions dialoge and no options just the close option. Can anybody help. I have uploaded a screen grab of what I see.

Thanks for that. Spooky really site seamingly works fine for a few weeks and then only when using advanced permissions do i find out that something must have gone wrong with the install. Luckily it is just a dev location so I just reinstalled C5 and now working fine.
