Blank page after form submission

Not sure what's going on.. but I have one client that if you check off to send the HTML form as an email it just goes to a blank page after you push submit. If you uncheck send email it works fine. Please advise...

I am pretty sure there is an error when sending and getting a fatal error on return. You should turn errors on and after the page sends, see if any errors are thrown by php (should display).
