Can't add Forms?

Need a bit of help.
Four websites updated to 5.4.2.1, but two wont allow me to add a form. Tried Extended form, basic built-in form and the survey block.
Get the pop up and I can fill title, pick type and check required, click add and nothing happens?
Anyone else had this?

This is serious now.
I have a lot of blocks that will not work?
Power Slider, Tags, form, Extended_form and Survey block.
Power block cannot add image?
Tag block no options?
Form, Extended form, Tag and Survey blocks allow input, but tabs do not work and when clicking Add all I get is a submit button?
