Can't get contact form to work

Hi there,
I have added a contact form on my webpage (www.petiquette.co.nz), but for some reason I can't get it to work. The verification commands show up, but the 'Thank you for your inquiry' doesn't appear after submitting the message, and the message does not send to my assigned email address.
Can somebody help me out and identify where I've gone wrong.
