Custom forgot password form

Hi I would like to have a page in my site where the user can fill in there emailadres to receive a email to change there password. Just like the standard loginpage except from the top login form. But I seems not to get it to work, any help would be appreciated.
Secondly I want to style the page '/login/change_password/somestring/', any ideas?
