Dasboarddatab reports ***PLEASE HELP***

I am currently trying to set up an external form on my website for users to submit information directly to the dashboard/database. I came across this forum on the web that shows how to do it.

Is this possible on Concrete5 or should I work with my web hosting company? If so I need someone to point me in the right direction on how to do it.
