I have the add-on...dealer search - 1.2. There seems to be two errors. First.....the form is too wide (even when I specified size). For example, the "include locations within" drop down field.....field goes all the way across the page.

Second....when I select option “show results and map”, the map doesn't appear. However, if I select the option "map only" ...I get the map.

What do I do to correct these errors?
