I put some files on my website for free download. But I want the visitors to leave their names and some other information before they can enter the download page. How should I do with concrete5? Thanks.

I am using Plain Yogurt theme. I created a Form containing several questions. I want the visitors fill out the forms and click Submit before they can enter the download page. They will be blocked if they try to dirctly link to the URL of the download page without filling out the form first. How should I do? Thanks.
it woud require some custom work, what you can do, is make it so that on form submit, it places a cookie, and if they try to navigate to that page, if the cookie exists let them in, if not, don't
