Error Adding Form Block to a Page

Hi,

I'm having an issue adding a form block to a page. Every time I try to save a new form after adding all the fields, I receive the attached error message. After I closed the error message, and I click on save again, I receive another error message. Both are attached.

This was happening on version 8.2.1, we upgraded to version 8.3.1 and the issues still exist.

Any thoughts/ideas?

I had a similar problem but I discovered that I reloaded the page after those error messages, I would find out my block was added anyway.

Your first error I am not sure I understand but your second error clearly indicates there was an attempt at adding a form that has already been added so my guess is your form is added despite the error you first see.

My suggestion is to close the form after the error message, reload the page and if you see the form there, publish the page.

Thank you for your response! Unfortunately in my case, the form was not saved after the error displayed. The second error (in my original post) seems like it is related to duplicate forms, however the form is not added to the page.

One of my field name is Address 1 (text field), and I'm wondering if this is causing the issue. However, I have tried adding a form with just an Address 1 (text field) field and that worked.

I had initially tried using the Address field and that was throwing another error (please see attached)

I found this post on github which seems to be related to the error I'm seeing. Doesn't look like there is a solution yet to this.
