Form block broken

Adding a form leads to the following error: 
Class 'MiniSurvey' not found


While still in edit mode, the area where the form should appear remains blank.

I'm working with a copy of the Elemental theme and all existing Javascript remains untouched.

What concrete5 version are you on? Are you using a template for the "form" block type, or just all default?

I'm running version 5.7.4.2 and using an Elemental default template within my application/themes directory.

I'm a little green so bear with me if I skimp on any details :)
