How do I set up contact page?

I can't seem to find the module that will have fields for the visitors to give their information and contact us through email?
its the form block. Its preloaded, you just need to fill in some info. To make it email you the results just go to the options part of it and select that and specify the email the results go to.

You might also want to check into some of the pro forms they are pretty nice
I have done as you describe (entered my email address under options) however, I am not receiving the emails. And yes, I double-checked that I entered the address correctly. Any other suggestions?
