Images are not showing

I am inserting images on page using the <img src= ""> but sometimes not always its as if the block isn't even there. When im in edit mode the block looks fully collapsed as well. I know I am using the correct code however it just randomly does this with some images. Sometimes they work for a while and then it just happens for no reason. Any suggestions to the source of the problem?
are you trying to do this in a template file or in a html block or something else?
well It actually happens with anything Im doing I can use any template provided by C5 and sometimes the block is just collapsed or it look right when im editing then when I go to Publish it disappears or get moved to a random position on the page.
But yes HTML blocks seem to have the most trouble. I use clean code as well so im not sure.
