No boxes to input credit card details

I've added the community store, and square integration. I have set up a product, which, then purchased moves the user to a new member level so they can view some pages. When i test it, I enter my address details in the store, the object is added to my cart, however, I am unable to enter any details as there are no boxes.

Is there something I have missed?
