Problems after trying to upgrade to 5.4.2

Something went wrong with our install so we returned to 5.4.1.1. Now we have 3 problems. The dashboard colors changed. The top interface (Edit Mode, Properties, etc.) has gone transparent. When we try to add a block, it comes up blank (no options) and no way to type.

Any ideas?

Update: We are able to change existing blocks, just can't make new ones.

Put it in Development and the error I'm getting is:

Fatal error: Cannot redeclare class LibraryFileBlockController in /usr/home/tra163/www/htdocs/concrete/blocks/library_file/controller.php on line 340

Found a solution for the block creation. Still have transparent interface and dashboard colors messed up.
