Unable to EDIT extended form ISSUES

Does anyone know how to resolve this issue: I created a form last year and now need to edit it to change dates/times etc. I am unable to edit. The page becomes unresponsive and grayed out in Concrete5 edit mode!

Suggestions, solutions and frustrations all welcome.
