URGENT Cant access Add Functionality from Dashboard 5.5

Error:
Fatal error: Cannot redeclare class FormBlockController in /var/www/vhosts/touch-akl.com/httpdocs/blocks/alt_form/controller.php on line 376
URGENT problem. I suddenly seem to have lost my connection with Add Functionlity either from Edit Page or the Dashboard.
Have no idea why this is but need to sort it quickly...I recently added a split layout for the first time then noticed the problem afterwards.
I cant say if it was caused by this or not though.
At the moment I just can't access this area and website feels broken.
It seems this url gives a blank white page

The other site I have upgraded to 5.4 is working fine...so far.

It seems my alternate template included the entire contents of the form block folder.
I deleted everything except the templates folder with the alternative view.php and view.css
Not sure why it suddenly went off though...
